Before Your Visit

What to prepare and bring for your visit

** Government Photo ID (and Alberta Health Care card if you have one).

A list of current medications (or bring the bottles) including the vitamins.**

Any relevant medical documents: lab/imaging results, discharge papers, specialist letters, and prior reports.**

** Your pharmacy name and address (helpful for sending prescriptions).

For children; immunization records if available, plus the child’s Health Card and a parent/guardian ID.**

** Walk-in during clinic hours and check in at the front desk.

To help us assess you faster, be ready to share your main concern, when it started, your symptoms, and any allergies or medications.**

If your concern is urgent or you’re feeling worse, let reception know right away so the care team can triage appropriately.**

Booked Appointments

How to prepare for a booked appointment

** Arrive 10-15 minutes early to allow time for check-in.

Write down your top 1-3 concerns and address what matters most first**

If you booked for a specific issue (forms, follow-up, recurring symptoms), bring any paperwork or past results with you.**

If you need a refill, bring your medication details (name, dose, and how often you take it).**

During Your Visit

What to expect during your visit at Fast MD

** Your provider will review your concern, medical history, and medications, then discuss an assessment and plan.

When needed, we may recommended tests, prescriptions, follow-up appointments, or a referral to a specialist.**

We aim to keep communication clear so you leave knowing your next steps**.

Emergency Care

Call 911 or go to the ER for emergencies

If you have chest pain, severe shortness of breath, signs of stroke, uncontrolled bleeding, severe allergic reaction, or any life-threatening symptoms, go to the nearest emergency department or call 911.
